Queen Creek Naturopathic, PLLC (“Queen Creek Naturopathic,” “QCN,” “we,” “us,” or “our”) respects your privacy. This Privacy Policy explains how we collect, use, disclose, and protect information when you visit queencreeknaturopathic.com (the “Website”), contact our office, submit a Website form, or follow a link from the Website to a third-party service.

Please read this Policy before using the Website. By using the Website, you acknowledge the practices described here. If you do not agree with this Policy, please do not use the Website.

1. The Short Version

  • The public Website is not the secure patient portal. Please do not submit detailed medical information through a general Website form, ordinary email, or social media.

This Policy applies to information collected through this Website and related communications with QCN. It does not replace QCN’s Notice of Privacy Practices, which governs how QCN may use and disclose protected health information in its role as a healthcare provider.

This Policy also does not control websites, applications, or services operated by third parties, even when we link to them. Examples may include the patient portal, scheduling or communications providers, Fullscript, social media platforms, Google, and Meta. Those services maintain their own privacy notices and terms.

Depending on how you interact with QCN, you may provide:

  • Your name, email address, telephone number, and preferred contact method;
  • Information included in an appointment request, contact form, newsletter signup, email, text message, or voicemail;
  • Basic information about the service you are interested in;
  • Communications, questions, feedback, or other information you choose to send; and
  • Information required to complete a transaction or administrative request, when applicable.

Do not include detailed medical history, diagnoses, medications, laboratory results, insurance information, Social Security numbers, payment-card information, or other highly sensitive information in a general Website form or ordinary email. Use QCN’s secure patient portal or call the office when sensitive information is necessary.

When you use the Website, QCN and its service providers may automatically collect information such as:

Meta Pixel

The Website uses the Meta Pixel supplied by Meta Platforms, Inc. The currently installed Pixel records page-view activity for advertising measurement. When the Pixel loads, Meta may receive information such as your IP address, browser or device information, the page visited, the referring page, cookie or advertising identifiers, and the date and time of the visit. If you use Facebook or Instagram, Meta may associate Website activity with your account in accordance with Meta’s policies and settings.

QCN does not intend to configure the Meta Pixel to collect the contents of Website forms, patient portal credentials, medical records, or the substance of communications with QCN. Nevertheless, because a visit to a healthcare Website may itself be sensitive, you should review your browser and Meta privacy settings and avoid entering sensitive medical information into public Website forms.

Do Not Track and Global Privacy Signals

Some browsers offer “Do Not Track” or other privacy signals. Because there is not yet a uniform standard that applies to every Website technology, this Website may not respond to every signal. 6. Healthcare Information and Tracking Technologies

QCN recognizes that privacy expectations are especially important in healthcare. The secure patient portal is separate from the public Website. Information submitted through the patient portal is governed by the portal provider’s terms, QCN’s Notice of Privacy Practices, and applicable healthcare privacy requirements.

The Website may link to services operated by other organizations, including the patient portal, Fullscript, social media networks, professional organizations, and outside resources. A link does not mean QCN controls or endorses a third party’s privacy practices. Review the privacy notice of each third-party service before submitting information.

9. Email, Text Messages, Telephone Calls, and Newsletters

If you contact QCN by email, text, or telephone, we may retain and use the communication to respond, schedule services, maintain records, and manage our relationship with you. Ordinary email and text messaging may not be secure. Do not use those channels for urgent matters or detailed medical information unless QCN instructs you to do so through an approved secure method.

If you subscribe to marketing email, you may unsubscribe using the link in a marketing message or by contacting QCN. Unsubscribing from marketing does not prevent appointment, care, billing, safety, or other non-marketing communications.

10. Data Retention

We retain information for as long as reasonably necessary for the purposes described in this Policy, including responding to requests, maintaining business and healthcare records, resolving disputes, enforcing agreements, protecting security, and satisfying legal, professional, tax, insurance, and regulatory obligations. The Website is intended for a general audience and is not directed to children under 13.
